Documentation inconsistencies slow everything down — patient care, compliance, billing, audits, and staff workflows.

⭐ About Jacqueline Swenson| Healthcare Documentation Consultant

Jacqueline Swenson brings over a decade of hands-on healthcare experience, working across medical billing, coding, and frontline hospital operations. Her work in emergency departments, specialty practices, and hospital billing offices has given her a rare, full-spectrum understanding of how documentation flows—and where it consistently breaks down.

She currently works as a Unit Secretary in a busy emergency department, supporting physicians, nurses, and clinical staff while navigating the fast-paced demands of Epic, Solarity, and other medical software. This frontline role gives her a daily view into how documentation habits impact communication, patient care, and the overall pace of the department.

Before moving into emergency care, Jacqueline spent years as a Billing Specialist and Medical Billing & Coding Specialist. She submitted claims, corrected documentation gaps, resolved denials, and tracked down the root causes of misprocessing and delays. She has seen firsthand how incomplete or inconsistent documentation affects reimbursement, compliance, and organizational trust.

Her early work as a Personal Care Attendant also grounded her in the human side of healthcare—patient dignity, clear communication, and the importance of systems that actually support care rather than complicate it.

Across every role, one pattern has been clear:
documentation inconsistencies cause avoidable chaos.

Jacqueline founded Vanguard Insight’s Healthcare Division to address that problem directly. Her work blends real-world clinical experience with practical operational structure, helping teams document clearly, consistently, and confidently.
